There is an entry in both databases although the unverified database may be better suited.

It is for "Premierplan" mobile phone insurance, as provided through Phones 4U. The registered address of the company is given at the bottom of the page as:

Osprey House
Ore Close
Lymedale Business Park
Newcastle Under Lyme
Staffordshire
ST5 9QD

This is the same address as Phones 4U.

However, the contact page gives the contact address as:

Policy Administration Services Limited
PO Box 290
Crewe
CW1 6WY

This PO Box goes to Lifestyle Services Group. I'm not sure whether they are linked, but it would appear that the phone numbers for Policy Administration Services are answered at Lifestyle's office in Crewe.

I have added the two 0844 numbers (0844 871 0600/0844 871 0524) to the entry. The number above, 01270 658000, was superceded by 01270 413000.
