If you could provide most or all of the details requested, then it would make our lives easier...

However, the answer is likely to be no. GP surgeries, and the providers of their rip-off numbers, are extremely protective of the underlying geographics and so it is highly unlikely we will be able to uncover it. You should complain to your GP about it.

I don't mind helping you with your request as long as you read the instructions!
