In answer to your question, I expect that you can contact them by telephone on the said 0845 number.

If, on the other hand, you are asking for help finding an alternative, then please start by reading the instructions and providing basic information and any other information you can find, rather than asking those you are asking to help you to spend additional time finding it.

It is my understanding that district nurses are generally based somewhere; often a GP surgery...

What is "NHS cut backs" and where is it based?

Entering "01922 district nurse" into Google gets a number of geographical hits:
