Sadly you have not provided US with much information...

there is a link at the top of the request page which tells you what information will help us.. apart from just the number.

I typed in Unite Financial Advice and got this link: http://www.unitetheunion.org/how-we-help/memberoffers/memberbenefits/unitefinanc...

As you can see it's operated by Independent Financial Matters... I did try the website given after the e-mail address: www.independentfinancialmatters.com but it gave me a 404 Error Page..

You could try e-mailing the address given on that page as a start...
